Adjutant Ltd ("we", "us", "our") operates YIHA (Your Intelligent Health Assessment). This page sets out two privacy policies: the first covers our marketing website, the second covers the YIHA platform where patients complete health assessments.

Part 1 — Website privacy policy

This policy covers the YIHA marketing website at yiha.health. It does not cover the YIHA platform where patients complete health assessments.

1. What we collect on this website

This site only collects information you actively submit through the contact form: your name, email address, phone number (optional), practice or organisation (optional), and the message itself. We do not collect patient data, health information or clinical details through this website.

2. How we use it and how long we keep it

When you submit the form, the details are forwarded by email to our team so we can reply to your enquiry. The form data is not stored in a database on this website. You can ask us to delete your information at any time, and we will act on the request promptly.

3. Cookies, analytics and tracking

We don't use cookies for advertising or cross-site tracking. We don't run third-party analytics scripts on this site, and we don't embed trackers that build a profile of visitors. Standard technical logs are kept for the purpose of operating the site securely, but they are not used to identify or profile you.

4. Third-party services

This website is hosted by a third-party provider, and the contact form is delivered via our email provider. These providers process form data on our behalf solely to deliver the message; they do not use it for their own purposes. We do not share your details with any other third party.

5. New Zealand Privacy Act 2020

Adjutant Ltd is a New Zealand-registered company and this website is operated from New Zealand. We handle personal information in accordance with the New Zealand Privacy Act 2020. If you have any concern about how your information has been handled, please contact us via our contact form and we will work to resolve it.

Part 2 — YIHA product privacy policy

This policy explains how we collect, use, store, and share personal and health information through the YIHA platform, in line with the Privacy Act 2020 and the Health Information Privacy Code 2020.

1. Who we are

Adjutant Ltd is a New Zealand-registered company that operates YIHA, a platform that lets patients complete health assessments online and have the results delivered to their GP practice.

When you complete an assessment through YIHA, your practice is responsible for the care they provide using the information YIHA delivers, and holds and manages your health information under their own privacy obligations once it is delivered. YIHA acts as their service provider for collecting and transmitting that information.

2. What information we collect

When you complete a health assessment, we collect your National Health Index (NHI) number and date of birth (used to identify you and match your assessment to your practice records, both are required to access a form), your health assessment answers, demographic details, and your name and contact details (such as email address and postcode).

When you register with a practice through YIHA, we collect the information needed to enrol you with a New Zealand general practice, including your name, date of birth, NHI, address, and enrolment details requested by the practice.

When practice staff use YIHA's portals, we collect their name, email address, role, and login credentials.

Automatically, we collect standard technical information (such as IP addresses) for security and service reliability.

3. Why we collect your information

We collect and use your information to:

Deliver your health assessment to your practice, as a PDF by email and, where configured, directly into their Medtech clinical system
Register or enrol you with a practice
Operate and maintain the service, including security monitoring
Bill practices for their use of YIHA
Improve the platform, using de-identified or aggregated information only

4. How your information is shared

With your practice. When you complete a health assessment, the results are delivered to the general practice that set up the form, by email or directly into their clinical system where configured. Once delivered, your practice holds and manages your health information under their own privacy obligations.

With our service providers. We use a small number of trusted providers to operate YIHA, including a New Zealand-based hosting provider and a payment processor for practice billing. These providers receive only the information necessary to perform their function.

We do not sell your information. We may disclose information to a New Zealand authority if lawfully required to do so.

5. Storage and security

We do not store identifiable patient health information outside New Zealand.

We take reasonable steps to protect your information from loss, unauthorised access, and misuse. Access to patient data is restricted to authorised staff, and practice staff can only see their own practice's data.

6. How long we keep your information

We do not keep your health information for longer than necessary.

Health assessment submissions are retained for a short period set by your practice, after which they are deleted from YIHA. Your practice retains their own copy under their own retention policies.

Registration information is retained for a limited period, after which personal identifiers are removed and only the approval audit trail is kept.

7. Your rights

Under the Privacy Act 2020 and the Health Information Privacy Code 2020, you have the right to:

Access your personal and health information held by us
Ask us to correct information that is inaccurate, out of date, or incomplete. If we do not agree, you may ask us to attach a statement of correction to your record.

To exercise these rights, contact us at privacy@adjutant.health. We may need to verify your identity before responding. We will respond within 20 working days, as required by law.

9. Cookies on the YIHA platform

YIHA uses essential cookies for the portals to function (such as keeping you logged in during a session). You can control cookies through your browser settings, though some portal features may not work if they are disabled.

10. Changes to this policy

We may update this privacy policy from time to time. When we make material changes, we will update the "last updated" date at the top of this page.
