The Patient Journey

1

Patient identified for review

Your team identifies a patient due for their annual review: asthma check, OCP review, HRT follow-up, or similar. Or the patient requests one themselves. YIHA works with scheduled recalls or ad-hoc requests.

Personalised link sent

A branded link is sent via text or email, using your practice domain if you wish. The patient opens it on their phone, tablet, or computer. No account creation needed. No app store.

Patient completes at home

The guided assessment takes 5-10 minutes. Questions are tailored to their condition: asthma control, BP readings, PHQ-9 for depression, medication checks, smoking and alcohol status. They can pause and resume if needed.

Data appears in Medtech

Results flow automatically into the patient's Medtech record: daily record, clinical inbox, screening tab (weight, height, BP, smoking, alcohol), and classifications. A structured PDF is generated for medico-legal completeness.

Clinician reviews and acts

You open the inbox item, review the data, and decide. For most stable patients, that's the end, with no appointment needed. For patients who need follow-up, you have the full picture before they walk through the door.

Deep Medtech Evolution Integration

YIHA writes directly into four areas of Medtech. Your team doesn't change their workflow. The data just appears where they expect it.

Clinical Inbox

Completed reviews appear in the patient's inbox, flagged for your attention. Open, review, action. Same workflow as any other clinical task.

Daily Record

A structured summary of the review is added to the daily record. Complete, auditable, and searchable alongside every other clinical entry.

Screening Tab

Weight, height, blood pressure, smoking status, alcohol use. All updated automatically, with no manual data entry into the screening dashboard.

Classifications

Smoking, vaping, and alcohol classifications are updated without a single click. Your QI data stays current without extra work.

NZ-Hosted. No AI. No Persistent Data Storage.

We built YIHA with the same security standards we expect for our own patients. Every decision was made with NZ health privacy requirements front of mind.

Secure patient health data stored on protected New Zealand servers

New Zealand Servers

All patient data is processed on secure Auckland servers. Nothing leaves New Zealand. No offshore processing. No cloud jurisdiction risks.

No AI Decision-Making

YIHA uses rule-based clinical logic developed and reviewed by experienced NZ GPs. There is no AI, no machine learning, no automated clinical decisions. The clinician is always in control.

No Persistent Personal Data

We do not store individual patient data beyond the time needed to process and deliver the assessment. Once the data is in your Medtech system, only anonymous aggregate data is retained for service improvement.

Structured PDF Backup

Every assessment generates a PDF summary for the patient record, independent of the Medtech integration. If your PMS changes in future, your data is portable.

The maths for a 5,000-patient practice

Two ways a typical practice can run YIHA. The conceptual case for each model lives on Why YIHA; the numbers below are what the day-to-day looks like in your books.

Efficiency-First: by the numbers

Same 5,000-patient practice running YIHA purely for time savings, no patient fee. One sentence of context, then the maths.

  • Subscription: $350 + GST / month (7ยข per patient)
  • Time returned: ~7 hours / week of clinical capacity
  • Reinvestment: freed slots go to acute care, complex patients, or protected clinician time
  • Net result: better access for patients who need to be seen, and a measurable brake on burnout

The subscription costs less than one locum session. The time it frees is worth multiples.

Revenue Model: by the numbers

Same practice choosing to charge a modest fee for remote reviews. The math at a glance:

Revenue/year (โ‰ˆ200 reviews/month ร— $5 ร— 12 months) $12,000
Subscription/year ($350 ร— 12) -$4,200
Net benefit +$7,800

On top of the 300-400 clinical hours freed, the modest patient fee can deliver a five-figure net annual benefit. Numbers are illustrative; talk to us about what they would look like for your patient mix.

Charging is entirely optional.

Email Option Available

If your practice uses a different Patient Management System, the standard email option delivers a structured PDF for each patient's completed review. Same clinical logic. Same price. You file it as you would any other clinical document.
